Grant Applications

Applications for Foundation funds are accepted annually between February 15 and April 15. No applications submitted after April 15 will be considered for the current award period. Applications may be mailed to the Foundation address or emailed to mail@sd113afoundation.org. Questions about an application may be submitted to the same email address and will be answered within 2 business days. Please use the links below to access an application.

Note, outside grant applications are accepted on a rolling basis and are subject to the granting body’s application guidelines and deadlines. The Foundation will periodically post available grant opportunities here and will also share through email. The form below is required to begin the process of seeking funds outside the Foundation budget.

Grant Evaluation Form

Grant evaluation forms are required for every awarded grant. Grant evaluation forms for the 2020-21 school year (awarded May, 2020) must be submitted as soon as the grant project is completed or by April 15th, 2021. New grant applications for Spring, 2020, will not be considered if the applicant has outstanding evaluations for the prior year.
